Abstract: Increasing numbers of youth with special health care needs are surviving into adulthood creating demands for improved services to facilitate their transition from pediatric to adult health care systems and other adult systems of care. In response, health care transition planning (HCTP) services and programs are being developed to respond to this need. Important to service and programmatic development is the advancement of science to generate the evidence needed for the provision of services. A key issue associated with the development and implementation of HCTP service models is the measurement of their effectiveness. A discussion of the issues associated with measuring the outcomes of HCTP effectiveness will be presented. Issues pertaining to HCTP theory development, design and methodology will be examined followed by recommendations for future directions.
