Abstract: Objective was to evaluate the use and perceived usefulness of a free online program that creates a portable health summary. Methods : General data was collected from all users of MyHealth Passport (www.sickkids.ca/myhealthpassport) between May 2007 to September 2008. Users were invited to fill out an online evaluation and/or give contact information for an in-depth telephone interview. Results: 742 users, median age 17 years. 66% were patients in our facility. 190 completed the online survey, 34 the phone survey. On both evaluations, the majority of users found the program easy to use. Most interviewees still have their passport and carry it most (71.4%) or some (14.3%) of the time. Interviews yielded four key elements: enjoyment, education, communication/navigation, ease of use. Conclusions: MyHealth Passport is easy to use, carried by most users and educates users while enhancing communication with health care providers.
