Abstract:
Sudden death is estimated to account for 250,000- 400,000 deaths per year in the United States. When this is due to a primary cardiac cause it is called Sudden Cardiac Arrest (SCA), which in most cases can be attributed to cardiovascular disease. In the absence of significant coronary artery disease (CAD) or manifest left ventricular dysfunction, a minority of SCA will be secondary to subclinical cardiomyopathy or primary electrical disease. Survivors can be a diagnostic and management challenge. A methodical assessment will be required to identify a diagnosis for the survivor and, in some cases, related family members, in order to prevent further events and decrease disease burden. Observational and provocative testing can reveal a cause in more than half of these cases. This review will outline a diagnostic approach to the SCA survivor when coronary artery disease is not thought to be the precipitating cause of the arrest event.
